Peoplechart raises the bar

Founded in 1999, Peoplechart has had a long and successful track record of requesting, obtaining, and integrating copies of medical records from multiple providers regardless of format (paper-based, electronically-transmitted, or self-uploaded files). Our full breadth of services combined with cutting-edge technology deliver a practical, cost-effective, value-added solution that bridges today's paper environment with electronic interoperability. The result is a comprehensive set of medical records that is portable with the patient.

In 2003, Peoplechart was one of twenty working group members of the initial phase of the Markle Foundation's Connecting-For-Health initiative. At the time, the charge was to define the attributes or characteristics of the Personal Health Record (PHR) model.

To date, we are proud of the support earned from consumer organizations such as the National Consumers League (NCL), National Alliance for Caregiving (NAC) and the National Council on Patient Information and Education (NCPIE). We serve as service provider of choice for several renowned medical universities and clinics across the country including the American College of Cardiology (disease management), Partners Healthcare (premiere service), Hawaii’s Queens Medical Center (breast cancer), University of Chicago Hospital (lung transplant), Benjamin Rose Institute (Ohio) (case management) and Transplant Institute of the Pacific (kidney transplant).
