Automated tracking: Dual focus on patient's adherence and outcomes

Multiple certifications with outpatient sources of data enable Peoplechart to track medication adherence without relying on input from the patient or from the EMR system. With electronic data feeds on prescription fills, lab results, and patient-reported feedback on non-adherence, Peoplechart can validate adherence in a way that most traditional EMR systems cannot achieve by themselves. Our focus on adherence, efficacy of medication therapy, and impact on health outcomes as reported through the lab data makes for a powerful solution towards optimal quality performance and reduced cost of care.

Multiple sources of sanctioned data

Peoplechart's data is sourced from a variety of service providers, including retail pharmacies, Pharmacy Benefit Managers (PBMs), diagnostic lab centers, and patients. MedsIncontext™ is Surescripts® certified for Prescription Benefit, Medication History and Prescription Routing services. Peoplechart is also a strategic partner of a third party lab data provider for the capture of patient's diagnostic lab results. Finally, our long-standing experience in soliciting patient's annotations and feedback through our enterprise-driven PHR system and our work in indexing paper medical records retrieved from multiple health providers contributes yet another important source of information about the patient.

Unifying patient information created in different formats

Recently, we received a patent for our method of integrating structured and unstructured data into a common format for display in a single interface that enables ease in review, search, and sort. Our ability to unify data from multiple formats ensures that the patient's information can be comprehensive and as important, accessible from the same page or category. Information that can be integrated using Peoplechart's technology includes electronic feeds, scanned images, file uploads, and self-entered data. This innovation is best illustrated in our MedsIncontext™ solution.

Customization in setting alerts for each patient or for a population of patients

For effective intervention, clinicians must be able to set (alerting) thresholds that are appropriate for each patient or for a population of patients. Peoplechart's applications are designed to provide clinicians with optimal flexibility – but not so much that preference setting becomes cumbersome. The ability for clinicians to set alerts by patient's specific thresholds was first used in the ACC CardioSmart Hypertension Tracker, powered by Peoplechart.

Integrated with most EHR systems

Peoplechart can accommodate a range of preference of our clients for lite to full integration. Our business logic layer is designed to support/generate object-oriented programming, Application Programming Interfaces (APIs), or iFrame technology. The key is seamless access whether clinicians are using a single integrated system or multiple independent systems. Whichever the preference, Peoplechart stands ready to help by providing resources, codes, and expertise.
