University of Chicago CMMI Project – Improving Inpatient/Outpatient Care

2012-2015

Peoplechart has been selected to participate in a study led by the University of Chicago Medical Center (UCMC) that aims to improve quality, outcome, and reduce the cost of hospitalizations, including readmissions. The Center for Medicare and Medicaid Innovations (CMMI) awarded over $6 Million in funding for the 3-year project. The study will implement and evaluate an innovative model of care in which "physicians lead a multi-disciplinary, team-based approach to care for patients identified with high risk of hospitalization, complex or multiple conditions, and who would normally fall outside of any disease-focused programs."

University of Chicago PHR Lung Transplant Project

2009-2010

Peoplechart provided medical record retrieval and classification services and a web-based application for access by hospital and clinical staff and by their recently lung-transplanted patients. The purpose of the collected information was to help staff members monitor patients for post-surgical, routine types of care.
