A Better Path to Addressing Poor Adherence

MedsIncontextTM offers a number of industry leading benefits and features that help address the $300BN poor adherence challenge.

Measure and Respond

Evidence-Based Algorithms and Mapping to Complex Data to Produce Actionable Results.

Data. MedsIncontextTM uses industry standard, evidence-based rules and medical ontologies that help define the relationships among source data sets. Fill-based adherence (behavioral data), prescriptions (treatment data), and lab results and biometrics that measure the progress of condition-based outcome, are all kept in proper context and assigned to specific, relevant conditions. The mapped data sets are then applied an absolute scale for rank-ordering of patients within sub-populations by user-selected or industry-default thresholds.

Actions. The translation of metrics such as normalized values to scores enable clinicians to compare risk levels of their patients by patient's behavior (adherence) and to further explore reasons for the poor adherence before moving to a change in prescribed treatment (such as dosage, substitution, or alternative therapy). The MedsIncontextTM scores can help…

  • Understand Risk Profiles of Patient or Sub-Populations – Access dynamic, interactive patient score cards that show how prescription fill-based adherence and condition-based lab results, biometrics have improved or declined over time: By single or multiple drugs, drug classes, or chronic conditions.
  • Evaluate Treatment Appropriateness – Determine whether prescribed treatment has improved outcomes by condition through drug-lab pairing – in a single-pane view, across time.
  • Understand "What Else Is At Play?" – The power behind the proprietary MedsIncontextTM 4-quadrant approach is its ability to raise questions about "What else at play" – hypotheses to consider before changing treatment.
  • Identify Populations with Different Types of Risks – Including polypharmacy issues (from co-morbidities), adverse interactions (from taking multiple classes of drugs), and unintentional side effects (from patient's self-reported data).
  • Automate Outbound Messaging – Save time for care team by auto-messaging patients when adherence falls below pre-set threshold. Provide clinicians with a head start by providing them with a history of patient's adherence, condition-based outcomes, and self-provided reasons for poor adherence.
  • Triage Patient Response – Review history of patient responses and rate of responsiveness before speaking with the patient. Triage patient to right staff member for more timely and appropriate outreach before any dialogue.

Visualize

Stunning Visuals that Follow Patient Across Time, Geography and Prescriber
  • Tools that build crosswalks for patients between in-network and out-of-network care
  • For directed care coordination, information on selected patient populations can be easily organized by provider, conditions, co-morbidities, and other categories.

Share of Care: MedsIncontextTM provides insights into patient decisions to seek care from both in-network and out-of-network providers by capturing prescribed treatment utilization by condition and location. Our geolocation mapping tells a visual story of healthcare providers with whom patients go for treatment: Prescriptions, labs and other services. This visual context is particularly useful for healthcare providers in value-based reimbursement programs.

Configured Rules, Thresholds, and Triggers Tailored to Profiles of Specific Patient Populations

Fully transparent drill-downs. We design each screen-view to save our clinicians time by providing informational context that help support essential decision-making. All results can be traced back to the underlying calculations based on industry standards and evidence-based guidelines. Guided drill-downs enable easy access and flexibility:

  • From summary results to detailed information
  • From individual patients to entire patient populations
  • From dashboard overviews to patient-level analytics
  • From within a category to across categories
  • From one data set to multiple data sets

All results can be traced back to the underlying, detailed calculations based on industry standards and evidence-based guidelines.

Configurable. Because every patient is different; every population is different; and every organizational workflows are different. Our out-of-the box system provides users with flexibility for clinicians to adjust the clinical rules, event notifications and thresholds to fit the specific profile, demographics and nuances of their population.

Other Features

Patient Adherence Confirmations

Our system sends automatic prompts by SMS text, HL7, ADT or by your PHR to patients for a secure response to request an explanation of the reasons behind any poor adherence. Patient responses and non-responses, along with informational context, can then be measured and triaged for intervention by the right staff member. Analytics about patient's response and responsiveness help your team develop the right adherence strategies across different populations. Our solution gives you the information you need to better align the identification of problems (risks) with the level and type of resources needed for the most efficient, high-touch intervention.

Certifications

MedsIncontextTM is certified by Surescripts® for the retrieval and display of prescription routing, formulary benefits, and medication history. It is also certified by Emdeon® for access and presentation of standardized lab results created by most major diagnostic centers across the country, including Labcorp®, Quest®, and other regional or hospital-based systems.

More than Patient Registry

Our medication adherence registry updates dynamically and enables efficient care coordination and follow-up. Review a list of prioritized patients at risk of poor adherence and poor outcomes at the condition level in a visually stunning way.

  • Polypharmacy issues (from having multiple chronic conditions)
  • Contraindications (in presence of certain conditions and classes of drugs)
  • Unintentional side effects (based on patient's self-reported data)

Optimize outcome at patient-level by understanding progression of condition by lab and biometric outcomes:

  • Lab and biometric results
  • Patterns of patient's fill behaviors
  • Pharmacotherapy issues
Interoperability and Data Exchange: HL7 / APIs

MedsIncontextTM supports industry standards for EHR integration. Our system can exchange data (both ways) with most health systems using HL7 CCDA, X12, XML, Json, CSV, Excel and comma delimited formats. You can easily export specific reports to us and we can provide channels for you to access our visual and table presentations. Meds-Incontext has over 25 different APIs available to request/send results.
